Гіполіпідемічна терапія і якість життя : фармакоекономічні аспекти

Abstract

The influence of the contemporary hypolipidemic therapy on the quality of life of the patients with the coronary atherosclerosis from the point of view of the evident medicine together with the side effects of hypolipidemic drugs affected this value has been reviewed. Among the contemporary hypolipidemic drugs much attention is paid to the most widespread classes, such as statins and fibrates. The hypolipidemic properties and the mechanisms of action of these drugs, as well as dynamics of some lipid metabolism values due to the therapy are given. Based on the results of the long-term multi-center trials the influence of the hypolipidemic therapy on patients' prognosis is discussed. The data about the influence of hypolipidemic drugs on the sensitivity of peripheral tissues to insulin and their efficacy in the primary and secondary prevention of cardio-vascular complications in patients with diabetes mellitus of type 2 are presented. The non-lipid-related effects of statins and fibrates and their role in the improvement of the quality of patients' life are taken a certain place in the review. The main side effects of the hypolipidemic therapy, which can decrease the quality of patients' life are also elucidated. The pharmacoeconomic advantages of the efficacy of hypolipidemic drugs compared with other interventions while treating atherosclerosis and its complications have been demonstrated. The question of the optimal choice of hypolipidemic drugs based on pharmaeconomics has been considered.

Висвітлений вплив сучасної гіполіпідемічної терапії на якість життя хворих на коронарний атеросклероз з позицій доказової медицини з урахуванням впливу на цей показник побічних ефектів гіполіпідемічних препаратів. Із сучасних гіполіпідемічних препаратів увагу приділено найбільш поширеним класам - статинам і фібратам. Представлені гіполіпідемічні властивості цих препаратів, їхні механізми дії, динаміка окремих показників ліпідного обміну під впливом лікування. На підставі результатів тривалих багатоцентрових досліджень обговорюється вплив гіполіпідемічної терапії на прогноз життя хворих. Наведені дані про вплив гіполіпідемічних засобів на чутливість периферичних тканин до інсуліну та їх ефективність у первинній і вторинній профілактиці серцево-судинних ускладнень у хворих на цукровий діабет ІІ типу. Певне місце в огляді відведене плейотропним ефектам статинів і фібратів, їх ролі в покращенні якості життя хворих. Також висвітлені найбільш важливі побічні ефекти гіполіпідемічної терапії, які можуть знижувати якість життя пацієнтів. Продемонстровані фармакоекономічні переваги ефективності гіполіпідемічних засобів порівняно з іншими втручаннями при лікуванні атеросклерозу та його ускладнень. Розглянуте питання оптимальності вибору гіполіпідемічних засобів з точки зору фармакоекономіки.
