Розлади жіночого здоров’я внаслідок вікового дефіциту естрогенів та їх корекція (огляд літератури)

Abstract

В огляді систематизовано сучасні дані щодо ролі естрогенів у регуляції різнома- нітних сторін життєдіяльності жіночого організму. Особливу увагу приділено головному статевому гормону естрадіолу, його геномним та негеномним ефектам, особливостям метаболізму та біологічної дії. Презентовані сучасні уявлення від- носно молекулярних механізмів дії естрадіолу, зокрема його впливу на різні типи естрогенових рецепторів. Описано зміни, що відбуваються внаслідок вікового, або іншої природи змен- шення продукції естрогенів. Приділено увагу механізмам розвитку порушень з боку сечостатевої, серцевосудинної, кісткової і нервової систем, метаболічних та гемостазіологічних ускладнень. Наведено сучасні дані про взаємозв’язок обміну статевих гормонів з іншими гормональними системами, вітаміном D, оксидатив- ним стресом тощо. Розглянуто сучасні уявлення про ефективність, користь і безпеку замісної гор- мональної терапії, різних лікарських форм та шляхів уведення препаратів, серед них продемонстровано особливості фармакодинаміки топічних вагінальних форм. Зазначено переваги своєчасного призначення препаратів менопаузальної терапії у період «відкритого терапевтичного вікна», у тому числі комбінованих гестоген/естрогенових лікарських засобів. Наведено результати багатоцентрових клінічних випробувань щодо ефективності та профілю безпеки естрогенвмісних препаратів та рекомендації до їх призначення, що ґрунтуються на засадах доказо- вої медицини. Показані перспективи розробки нових лікарських засобів для фармакологічної корекції гіпоестрогенових станів різної етіології.

This review presents in a systemized fashion the current available information as to the role of estrogen in the regulation of various aspects of the vital activity of the female body. Particular attention is paid to the main sex hormone – estradiol, its genomic and non-genomic effects, features of metabolism and biological action. Contemporary representations concerning the molecular mechanisms of its action, in particular its effect on various types of estrogen receptors, are presented. Also, we describe the changes that occur as a result of age, or other natural causes that result in reduced estrogen production. Special attention is paid to the mechanisms of the development of disorders of the genitourinary, cardiovascular, bone and nervous systems, metabolic and hemostasis-related diseases, and their complications. Modern data on the interconnection of sex hormones with other hormonal systems, vitamin D, oxidative stress, etc. are presented. Contemporary representations about efficiency, benefits and the hazards of substitutional hormonal therapy, including its various dosage forms and the various ways of drug administration are considered; among them the features of pharmacodynamics of topical vaginal forms are also considered. IN addition, we discuss the advantages of the proper timing of determining the onset of menopause, and therefore the administration of the appropriate prescribe therapies, such as combined gestogen / estrogen medications, are noted. The results of multicenter clinical trials on the effectiveness and profile of the safety of estrogen-containing drugs and recommendations for their appointment, based on the principles of evidence-based medicine, are presented. Finally, prospective new drugs designed for the pharmacological treatment of hypoestrogen syndrome of differing etiologies is examined.
