Краткий осмотр (реферат): У дисертації обґрунтовано доцільність оптимізації терапії ХХН шляхом застосування комбінацій похідних ГА з кверцетином у лікарських формах для перорального та ін’єкційного введення у залежності від характеру та стадії захворювання. За результатами досліджень доведено, що для перорального застосування при ХХН найдоцільнішим є комбінований препарат Глюквамін у формі капсул, який містить ГА г/х, N-аГА та кверцетин у співвідношенні 3:3:2, а при ін’єкційному введенні – комбінація N-аГА та КОР у співвідношенні 1:1. Визначено показники ЕД50 даних об’єктів, які склали 79,7 мг/кг для Глюкваміну та 30,2 мг/кг для комбінації N-аГА/КОР. Доведено їх високу безпеку як засобів лікування захворювань нирок при курсовому застосуванні. Оцінка ефективності на різних моделях ниркової патології показала, що Глюквамін (капсули) є найбільш перспективним препаратом при лікуванні ХХН І-ІІІ ст., особливо діабетичного та аутоімунного походження за умов латентного перебігу, а комбінація N-аГА/КОР (ін’єкції) – при ХХН IІІ-V ст., особливо за умов розвитку ТНН й кардіоваскулярних ускладнень, зокрема у сполученні з Глюкваміном, та у якості екстреної допомоги при ішемії нирок, ГУН та загостреннях ХХН. У механізмах нефропротекції досліджуваних об’єктів доведено вагому роль компенсації пластичної недостатності ниркової тканини, інгібування активності медіаторів запалення, активації та адгезії лейкоцитів, відновлення ендотеліальної функції та пригнічення вазоконстрикції, покращення ниркової гемодинаміки й реологічних властивостей крові, запобігання процесів апоптозу й проліферації, гальмування окисного стресу, відновлення балансу ниркової регуляції РААС. Комплекс отриманих результатів обґрунтовує доцільність створення комбінованих препаратів похідних ГА з кверцетином та їх впровадження в лікування ХХН.
В диссертации обоснована целесообразность оптимизации терапии хронической болезни почек (ХБП) путем применения комбинаций производных ГА с кверцетином в лекарственных формах для перорального и инъекционного введения в зависимости от характера и стадии заболевания. Результаты исследований доказали, что для перорального применения при ХБП целесообразным является комбинированный препарат Глюквамин в форме капсул, содержащий ГА г/х, N-аГА и кверцетин в соотношении 3:3:2, а при инъекционном введении – комбинация N-аГА и КОР в соотношении 1:1. Определены показатели ЭД50 данных объектов, составляющие 79,7 мг/кг для Глюквамина и 30,2 мг/кг для комбинации N-аГА/КОР. Доказана их высокая безопасность как средств лечения заболеваний почек при курсовом применении. Оценка эффективности на разных моделях нефропатий показала, что Глюквамин (капсулы) является наиболее перспективным препаратом в терапии ХБП I-III ст., особенно диабетического и аутоиммунного происхождения при латентном течении, а комбинация N-аГА/КОР (инъекции) – ХБП III-V ст., особенно при терминальной почечной недостаточности и кардиоваскулярных осложнениях, в том числе в сочетании с Глюквамином, и в качестве экстренной помощи при остром поражении, ишемии почек и обострениях ХБП. В механизмах нефропротекции исследуемых объектов доказана весомая роль компенсации пластической недостаточности почечной ткани, ингибирования активности медиаторов воспаления, активации и адгезии лейкоцитов, восстановления эндотелиальной функции и угнетения вазоконстрикции, улучшения почечной гемодинамики и реологических свойств крови, предотвращения процессов апоптоза и пролиферации, торможения окислительного стресса, восстановления баланса почечной регуляции РААС. Комплекс полученных результатов обосновывает целесообразность создания комбинированных препаратов производных ГА с кверцетином и их внедрения в лечение ХБП.
The thesis is devoted to theoretical generalization and a new practical solution of the scientific problem of optimization of the chronic kidney disease (CKD) treatment. The dissertation substantiates the possibility to increase efficacy and safety of CKD treatment by using combinations of glucosamine (GA) derivatives with quercetin in dosage forms for oral and injectable administration (depending on the stage of CKD) and proves expediency of development and clinical implementation of nephroprotective drugs based on these substances. The study determined that the combination of GA hydrochloride, N-acetylglucosamine (N-aGA) and quercetin in the ratio of 3:3:2 is the most expedient for oral use in CKD treatment. A comparative efficacy study of this combination in different dosage forms for intragastric (i.g.) administration showed that, in capsules (as medication "Gluquamine"), it shows a pronounced nephroprotective effect in minimal change disease and chronic renal failure (CRF) in a magnitude that significantly exceeds the activity of the tablet form of this combination and the reference drugs Quertin and Lespefril. Also, it was shown that N-aGA is the most appropriate component for the development of an injectable drug for CKD treatment because it was superior to GA hydrochloride at intramuscular (i.m.) administration in terms of efficacy in minimal change disease, showed a significant nephroprotective effect in acute kidney injury (AKI) without differences from intravenous (i.v.) administration, as well as in CRF with a pronounced antioxidant effect. This amino sugar should be combined with Corvitin (COR) (which has a prolonged pharmacokinetic profile at i.m. administration relative to i.v. use and a bioavailability of 93%) in a ratio of 1:1, as was confirmed by the highest efficacy of the N-aGA/COR (1:1) combination in AKI, which significantly exceeded the activity of other combinations. The studies concluded that ED50 of Gluquamine is 79.7 mg/kg (19 mg/kg or 1330 mg/day for a human) and ED50 of N-aGA/COR combination is 30.2 mg/kg (7.2 mg/kg or 503 mg/day for a human). Safety studies of Gluquamine and the N-aGA/COR combination as treatment for renal pathology showed that, when used in doses 3 times higher than ED50, they did not show any toxic effects in rats of both sexes, contributed to increasing kidney functional reserve and reduced azotemia. The N-aGA/COR combination did not have any significant local irritant effect on the muscle tissue during the course of i.m. injections and, like Gluquamine, had LD50 of more than 5000 mg/kg, allocating both of these substances to the VI toxicity class – relatively harmless substances. The results of Gluquamine (80 mg/kg, i.g.) efficacy study in diabetic nephropathy showed that prophylactic treatment with Gluquamine had a positive effect on the nephropathy, reduced animal mortality, had a significant nephroprotective, hypoglycemic and antioxidant effects, exceeding (p<0.05) the efficacy of comparators Quertin and Lespefril. Gluquamine (80 mg/kg, i.g.) also showed a high level of efficacy on a model of autoimmune glomerulonephritis. It significantly improved renal excretory function, suppressed nephrotic and urinary syndromes, normalized protein metabolism, had a hypoazotemic and an antiplatelet effect, reduced symptoms of renal anemia and nonspecific hematological manifestations of immune inflammation. The studies demonstrated that it increases the kidney content of endogenous N-aGA and its free blood fraction, which indicated the recovery of the damaged kidneys membranes. The results of enzyme immunoassays showed that Gluquamine improved vascular endothelium function, reduced hypoxia, normalized the balance of vasodilatation and vasoconstriction factors, as well as reduced kidney content of eicosanoids. In addition, Gluquamine had a significant nephroprotective effect, preserving kidney ultra- and histostructure as well as reducing the degree of glomerulosclerosis and tubular injury. The immunohistochemical study revealed that Gluquamine had a pronounced antiproliferative effect and reduced the intensity of apoptosis. Moreover, taking in account all the studied data set, Gluquamine exceeded (p<0.05) the efficacy of the reference drugs Quertin and Lespefril. In a comparative study of the experimental objects’ efficacy in tubular CRF, N-aGA/COR combination (30 mg/kg, i.m.) showed the highest efficacy level, as evidenced by zero animal mortality, improvement of renal excretory function, a decrease in tubular injury markers, normalization of electrolytes excretion and phosphorus-calcium metabolism. Moreover, according to most of the studied indicators, the combination exceeded (p<0.05) Gluquamine and the reference drugs Corvitin and Lespefril. Under the conditions of end-stage renal disease, the N-aGA/COR combination (30 mg/kg, i.m.) also showed the highest level of efficacy, manifested by an improved kidney functional state, a pronounced hypoazotemic effect, and a reduction of renal hypertension and anemia. Additionally, the combination showed high cardioprotective activity, prevented development of cardio-renal syndrome, heart failure with hypertrophy and diastolic myocardial dysfunction, which was confirmed by normalization of ECG parameters and histomorphological results. Moreover, taking in account all the studied data set, the combination exceeded (p<0.05) Gluquamine and reference drug COR. The results of the study of N-aGA/COR combination (30 mg/kg, i.v.) efficacy in ischemic ARI showed that it significantly enhanced kidney functional reserve under the water load, restored kidney functional state and concentration ability under the conditions of spontaneous diuresis, and significantly enhanced renal hemodynamics. It exceeded (p<0.05) the efficacy of comparator COR in most of the studied indicators. Generalization of the obtained data concludes that important links in the mechanism of nephroprotection of the studied objects include: compensation of plastic insufficiency of kidney membranes; inhibition of proinflammatory mediators and factors of leukocyte activation and adhesion; recovery of vascular endothelial function; suppression of renal vasoconstriction; an increase of renal hemodynamics due to the endothelial mechanism of regulation; improvement of blood rheological properties; slowing down proliferative processes, prevention of nephrocyte apoptosis, inhibition of renal oxidative stress; restoration of the balance in renal regulation of renin-angiotensin-aldosterone system. The results of the dissertation substantiate the expediency of using the combined drugs containing GA derivatives with quercetin to optimize treatment of renal pathology: while Gluquamine (capsules) is a promising drug for treatment of I-III stages of CKD (especially latent form), the N-aGA/COR combination is appropriate for treatment of III-V stages of CKD, especially in cardiovascular complications (N-aGA/COR can be used in combination with Gluquamine as well), and as an emergency treatment for ARI and CKD exacerbations. These results need to be confirmed in appropriate clinical trials
